    Modèle possibiliste pour la reconnaissance d'activités habitat intelligent

    Le vieillissement actuel de la population provoque un accroissement de problèmes dans les systèmes de santé, dont une pénurie de personnel médical pour les soins à domicile. Le vieillissement de la population a également pour effet d'augmenter le nombre de personnes avec troubles cognitifs. Les comportements incohérents induits par les symptômes des troubles cognitifs limitent la capacité de ces personnes à réaliser leurs activités de la vie quotidienne (AVQ). L'un des axes de recherche prometteurs de cette problématique est l'amélioration et le maintien de la qualité de vie des personnes avec troubles cognitifs dans leurs domiciles. Pour répondre à cette problématique, plusieurs laboratoires de recherche, dont le laboratoire de DOmotique et d'informatique Mobile de l'Université de Sherbrooke (DOMUS), explorent les différents moyens de soutenir, à l'intérieur d'un habitat intelligent, un occupant avec troubles cognitifs dans l'accomplissement de ses AVQ. Cette approche s'inscrit dans le récent courant de pensée issu de l'intelligence ambiante, qui fait référence à une tendance où les environnements sont enrichis avec des technologies (capteurs, effecteurs et autres dispositifs interconnectés par un réseau), dans le but de concevoir un système pouvant planifier une assistance ponctuelle aux occupants en fonction des informations recueillies et de l'historique des données accumulées. L'une des difficultés majeures inhérentes à ce contexte est la reconnaissance et la prédiction des comportements anormaux lorsque les occupants effectuent leurs AVQ à l'intérieur d'un habitat intelligent. Cette thèse vise à contribuer à l'amélioration du processus de reconnaissance de comportements d'un occupant avec troubles cognitifs. Notre proposition consiste en une approche de reconnaissance et prédiction de comportements fondée sur une formalisation des actions basée sur la théorie des possibilités, une alternative à la théorie des probabilités. Les actions sont inférées à partir de l'état actuellement observé de l'habitat intelligent obtenu grâce aux évènements envoyés par les capteurs présents dans l'appartement, lesquelles peuvent fournir une information incomplète et imparfaite. À partir de la séquence d'actions observées plausibles, l'approche proposée utilise une formalisation des activités en structure de plans d'actions pour inférer le comportement observé de l'occupant. Cette approche est en mesure de considérer les comportements erronés, où l'occupant effectue de façon erronée certaines activités tandis que d'autres peuvent être effectuées de façon cohérente, et les comportements cohérents, où l'occupant effectue une ou plusieurs activités de façon cohérente. Les hypothèses sur le comportement observé sont ensuite utilisées pour déterminer les opportunités d'assistance que l'habitat intelligent peut offrir. L'approche proposée a été implémentée et validée au sein de l'infrastructure du projet"Ambient Intelligence for Home-based Elderly Care" à l'"Institute for Infocomm Research" de Singapour et présente des résultats prometteurs pour des scénarios de cas réels effectués dans l'infrastructure. Le développement d'un habitat intelligent capable de maintenir et d'améliorer la qualité de vie des personnes avec troubles cognitifs permettrait de diminuer le fardeau des aidants naturels et professionnels, facilitant le choix des ces personnes de rester à domicile. Ce type de technologie pourrait constituer une solution viable aux problèmes des systèmes de santé associés au vieillissement de la population. De plus, ce type d'approche peut également être utilisé dans des contextes où les comportements anormaux et les situations à risque doivent être évités comme, par exemple, dans le domaine de l'aviation

    Six-minute walk distance after coronary artery bypass grafting compared with medical therapy in ischaemic cardiomyopathy

    Background: In patients with ischaemic left ventricular dysfunction, coronary artery bypass surgery (CABG) may decrease mortality, but it is not known whether CABG improves functional capacity. Objective: To determine whether CABG compared with medical therapy alone (MED) increases 6 min walk distance in patients with ischaemic left ventricular dysfunction and coronary artery disease amenable to revascularisation. Methods: The Surgical Treatment in Ischemic Heart disease trial randomised 1212 patients with ischaemic left ventricular dysfunction to CABG or MED. A 6 min walk distance test was performed both at baseline and at least one follow-up assessment at 4, 12, 24 and/or 36 months in 409 patients randomised to CABG and 466 to MED. Change in 6 min walk distance between baseline and follow-up were compared by treatment allocation. Results: 6 min walk distance at baseline for CABG was mean 340±117 m and for MED 339±118 m. Change in walk distance from baseline was similar for CABG and MED groups at 4 months (mean +38 vs +28 m), 12 months (+47 vs +36 m), 24 months (+31 vs +34 m) and 36 months (−7 vs +7 m), P>0.10 for all. Change in walk distance between CABG and MED groups over all assessments was also similar after adjusting for covariates and imputation for missing values (+8 m, 95% CI −7 to 23 m, P=0.29). Results were consistent for subgroups defined by angina, New York Heart Association class ≥3, left ventricular ejection fraction, baseline walk distance and geographic region. Conclusion: In patients with ischaemic left ventricular dysfunction CABG compared with MED alone is known to reduce mortality but is unlikely to result in a clinically significant improvement in functional capacity

    Screening Methodology for the Efficient Pairing of Ionic Liquids and Carbonaceous Electrodes Applied to Electric Energy Storage

    A model is presented that correlates the measured electric capacitance with the energy that comprises the desolvation, dissociation and adsorption energy of an ionic liquid into carbonaceous electrode (represented by single-wall carbon nanotubes). An original methodology is presented that allows for the calculation of the adsorption energy of ions in a host system that does not necessarily compensate the total charge of the adsorbed ions, leaving an overall net charge. To obtain overall negative (favorable) energies, adsorption energies need to overcome the energy cost for desolvation of the ion pair and its dissociation into individual ions. Smaller ions, such as BF4 −, generally show larger dissociation energies than anions such as PF6 − or TFSI−. Adsorption energies gradually increase with decreasing pore size of the CNT and show a maximum when the pore size is slightly greater than the dimensions of the adsorbed ion and the attractive van der Waals forces dominate the interaction. At smaller pore diameters, the adsorption energy sharply declines and becomes repulsive as a result of geometry deformations of the ion. Only for those diameters where the adsorption reaches maximum values is the adsorption energy sufficiently negative to balance the positive dissociation and desolvation energies. We present for each ion (and ionic liquid) what the most adequate electrode pore size should be for maximum capacitance

    Synbiotic approach restores intestinal homeostasis and prolongs survival in leukaemia mice with cachexia

    Cancer cachexia is a multifactorial syndrome that includes muscle wasting and inflammation. As gut microbes influence host immunity and metabolism, we investigated the role of the gut microbiota in the therapeutic management of cancer and associated cachexia. A community-wide analysis of the caecal microbiome in two mouse models of cancer cachexia (acute leukaemia or subcutaneous transplantation of colon cancer cells) identified common microbial signatures, including decreased Lactobacillus spp. and increased Enterobacteriaceae and Parabacteroides goldsteinii/ASF 519. Building on this information, we administered a synbiotic containing inulin-type fructans and live Lactobacillus reuteri 100-23 to leukaemic mice. This treatment restored the Lactobacillus population and reduced the Enterobacteriaceae levels. It also reduced hepatic cancer cell proliferation, muscle wasting and morbidity, and prolonged survival. Administration of the synbiotic was associated with restoration of the expression of antimicrobial proteins controlling intestinal barrier function and gut immunity markers, but did not impact the portal metabolomics imprinting of energy demand. In summary, this study provided evidence that the development of cancer outside the gut can impact intestinal homeostasis and the gut microbial ecosystem and that a synbiotic intervention, by targeting some alterations of the gut microbiota, confers benefits to the host, prolonging survival and reducing cancer proliferation and cachexia

    Long-Term Alterations of Cytokines and Growth Factors Expression in Irradiated Tissues and Relation with Histological Severity Scoring

    PURPOSE: Beside its efficacy in cancer treatment, radiotherapy induces degeneration of healthy tissues within the irradiated area. The aim of this study was to analyze the variations of proinflammatory (IL-1α, IL-2, IL-6, TNF-α, IFN-γ), profibrotic (TGF-β1), proangiogneic (VEGF) and stem cell mobilizing (GM-CSF) cytokines and growth factors in an animal model of radiation-induced tissue degeneration. MATERIALS AND METHODS: 24 rats were irradiated unilaterally on the hindlimb at a monodose of 30 Gy. Six weeks (n=8), 6 months (n=8) and 1 year (n=8) after irradiation the mediators expression in skin and muscle were analyzed using Western blot and the Bio-Plex® protein array (BPA) technology. Additional histological severity for fibrosis, inflammation, vascularity and cellularity alterations scoring was defined from histology and immnunohistochemistry analyses. RESULTS: A significant increase of histological severity scoring was found in irradiated tissue. Skin tissues were more radio-sensitive than muscle. A high level of TGF-β1 expression was found throughout the study and a significant relation was evidenced between TGF-β1 expression and fibrosis scoring. Irradiated tissue showed a chronic inflammation (IL-2 and TNF-α significantly increased). Moreover a persistent expression of GM-CSF and VEGF was found in all irradiated tissues. The vascular score was related to TGF-β1 expression and the cellular alterations score was significantly related with the level of IL-2, VEGF and GM-CSF. CONCLUSION: The results achieved in the present study underline the complexity and multiplicity of radio-induced alterations of cytokine network. It offers many perspectives of development, for the comprehension of the mechanisms of late injuries or for the histological and molecular evaluation of the mode of action and the efficacy of rehabilitation techniques

    Frequency, prognostic impact, and subtype association of 8p12, 8q24, 11q13, 12p13, 17q12, and 20q13 amplifications in breast cancers

    BACKGROUND: Oncogene amplification and overexpression occur in tumor cells. Amplification status may provide diagnostic and prognostic information and may lead to new treatment strategies. Chromosomal regions 8p12, 8q24, 11q13, 17q12 and 20q13 are recurrently amplified in breast cancers. METHODS: To assess the frequencies and clinical impact of amplifications, we analyzed 547 invasive breast tumors organized in a tissue microarray (TMA) by fluorescence in situ hybridization (FISH) and calculated correlations with histoclinical features and prognosis. BAC probes were designed for: (i) two 8p12 subregions centered on RAB11FIP1 and FGFR1 loci, respectively; (ii) 11q13 region centered on CCND1; (iii) 12p13 region spanning NOL1; and (iv) three 20q13 subregions centered on MYBL2, ZNF217 and AURKA, respectively. Regions 8q24 and 17q12 were analyzed with MYC and ERBB2 commercial probes, respectively. RESULTS: We observed amplification of 8p12 (amplified at RAB11FIP1 and/or FGFR1) in 22.8%, 8q24 in 6.1%, 11q13 in 19.6%, 12p13 in 4.1%, 17q12 in 9.9%, 20q13(Z )(amplified at ZNF217 only) in 9.9%, and 20q13(Co )(co-amplification of two or three 20q13 loci) in 8.5% of cases. The 8q24, 12p13, and 17q12 amplifications were correlated with high grade. The most frequent single amplifications were 8p12 (9.8%), 8q24 (3.3%) and 12p13 (3.3%), 20q13(Z )and 20q13(Co )(1.6%) regions. The 17q12 and 11q13 regions were never found amplified alone. The most frequent co-amplification was 8p12/11q13. Amplifications of 8p12 and 17q12 were associated with poor outcome. Amplification of 12p13 was associated with basal molecular subtype. CONCLUSION: Our results establish the frequencies, prognostic impacts and subtype associations of various amplifications and co-amplifications in breast cancers

    Differences in the carcinogenic evaluation of glyphosate between the International Agency for Research on Cancer (IARC) and the European Food Safety Authority (EFSA)

    The International Agency for Research on Cancer (IARC) Monographs Programme identifies chemicals, drugs, mixtures, occupational exposures, lifestyles and personal habits, and physical and biological agents that cause cancer in humans and has evaluated about 1000 agents since 1971. Monographs are written by ad hoc Working Groups (WGs) of international scientific experts over a period of about 12 months ending in an eight-day meeting. The WG evaluates all of the publicly available scientific information on each substance and, through a transparent and rigorous process,1 decides on the degree to which the scientific evidence supports that substance's potential to cause or not cause cancer in humans. For Monograph 112,2 17 expert scientists evaluated the carcinogenic hazard for four insecticides and the herbicide glyphosate.3 The WG concluded that the data for glyphosate meet the criteria for classification as a probable human carcinogen. The European Food Safety Authority (EFSA) is the primary agency of the European Union for risk assessments regarding food safety. In October 2015, EFSA reported4 on their evaluation of the Renewal Assessment Report5 (RAR) for glyphosate that was prepared by the Rapporteur Member State, the German Federal Institute for Risk Assessment (BfR). EFSA concluded that ?glyphosate is unlikely to pose a carcinogenic hazard to humans and the evidence does not support classification with regard to its carcinogenic potential?. Addendum 1 (the BfR Addendum) of the RAR5 discusses the scientific rationale for differing from the IARC WG conclusion. Serious flaws in the scientific evaluation in the RAR incorrectly characterise the potential for a carcinogenic hazard from exposure to glyphosate. Since the RAR is the basis for the European Food Safety Agency (EFSA) conclusion,4 it is critical that these shortcomings are corrected

    Meta-analysis of epigenome-wide association studies in neonates reveals widespread differential DNA methylation associated with birthweight

    Birthweight is associated with health outcomes across the life course, DNA methylation may be an underlying mechanism. In this meta-analysis of epigenome-wide association studies of 8,825 neonates from 24 birth cohorts in the Pregnancy And Childhood Epigenetics Consortium, we find that DNA methylation in neonatal blood is associated with birthweight at 914 sites, with a difference in birthweight ranging from -183 to 178 grams per 10% increase in methylation (P-Bonferroni <1.06 x 10(-7)).
